PITTSBURGH - eTradeWire -- Ten Ten Ten, a nonprofit organization dedicated to improving U.S. health outcomes and reducing healthcare costs, today announced the release of Maximizing Clinical Operations: The Operating System of a Value-Based Primary Care Practice, a new implementation guide for independent primary care practices.

The 89-page guide helps practices turn value-based care goals into daily work. It covers patient empanelment and payer attribution, working at the top of each team member's scope, pre-visit planning, morning huddles, follow-up on results and referrals, transitions of care, and patient outreach. Workbook exercises help teams adapt workflows to their practice.

"Value-based care succeeds or fails in the work between visits," said Jenn Block, PhD, Ten Ten Ten cofounder and Chief Architect, Value-Based Care. "A patient needs a follow-up call after discharge. A referral needs to reach the specialist and come back with a plan. This guide gives practices a way to assign that work, track it, and make sure it gets done."

The guide begins by reconciling payer-attributed patients with those the practice actually sees. It then shows how to prepare for visits, coordinate the team, and assign ownership of test results, orders, referrals, and patient messages. A transitions-of-care workflow sets out steps following hospital discharge.

To sustain these routines, Maximizing Clinical Operations provides a daily, weekly, and monthly operating schedule and a dashboard tracking pre-visit preparation, referral completion, and post-discharge outreach. A Plan-Do-Study-Act worksheet helps teams test one workflow before expanding it.

"From there, the practice needs systems that reveal what has been missed and make the next action clear. The goal is dependable care that does not rely on someone remembering every task."
