Integrated network matches at-risk Texas community with unique prevention program
PLANO, Texas - Feb. 12, 2019 - eTradeWire -- Solera Health, a leading integrated community network, added Fit for Service Wellness in Plano, Texas to its national network of Centers for Disease Control and Prevention (CDC)-recognized diabetes prevention program (DPP) providers. Eighty-six million American adults are living with prediabetes and 90 percent of them are unaware they are at high risk of developing type 2 diabetes. To combat this epidemic, diabetes prevention programs recognized by the CDC work with people with prediabetes on lifestyle changes that will reduce their risk of developing type 2 diabetes and improve their overall health. The Fit for Service Wellness DPP focuses on community support to help participants instill lasting healthy lifestyle changes. The program encourages participants to bring friends and family to DPP classes and offers grocery store tours and cooking demonstrations as well as exercise tips for increased physical activity. Additionally, individual coaching is available to guide participants through any challenges they may experience. The DPP has been going strong for four years and boasts many success stories including one participant who initially lost 50 pounds, but whose continued lifestyle change was so significant, shed a total of 80 pounds at last tally. "Since we can't change our past, we can only affect our future" said Pamela Nelson, RN, PhD, president and CEO of Fit for Service Wellness. "Solera Health will help us ensure a time when Plano is diabetes-free by not only growing our pool of DPP participants, but also giving us the crucial regulatory support to navigate Medicare compliance."
