The newly renovated site, located at 1901 S. Congress Ave., has been redesigned to meet the growing demand for adult primary care services. With state-of-the-art facilities and an expert team of healthcare professionals, FoundCare aims to enhance the patient experience and ensure that individuals have another access point for high-quality care.
FoundCare recognizes the importance of accessible primary care in promoting overall health and well-being. Through this expansion, the organization strives to address the evolving healthcare needs of the community by offering a wide range of services, including adult primary care, chronic disease management, on-site laboratory, behavioral health, and medical case management.

FoundCare CEO Christopher Irizarry expressed enthusiasm about the expansion and its potential impact on the community. Services will be available Monday through Friday from 8 a.m. to 5 p.m.
